COLLEGE PARK DOG PARK ASSOCIATION

MEMBERSHIP APPLICATION AGREEMENT

Membership in the College Park Dog Park Association is contingent upon a member's agreement with the terms listed below.  Prospective members are asked to agree with these terms at the time of application for membership and during renewal of membership.

The cost of the annual membership is $30 for the first dog in a household and $10 for each additional dog (for example, the fee for three dogs would be $50). You will receive a permit tag, valid only for the above dog(s), and the park gate lock combination, after you have paid for your annual membership.

Note: During the year the park may be closed for maintenance and special events.

In consideration of the acceptance of this registration at the College Park Dog Park, I agree that College Park Dog Park Association, Inc. has the right to refuse or cancel this registration at any time, for any cause which the members of College Park Dog Park Association, Inc. shall deem to be sufficient.

I agree to indemnify and hold College Park Dog Park, Inc. (the “CPDPA”) and its board of directors and the Maryland-National Park and Planning Commission (MNCPPC), harmless from any claim of loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by me or the act of my dog while in or upon the premises or grounds near any entrance thereto, and I personally assume all responsibility and liability for any such claims, and I further agree to hold the aforementioned parties harmless from any claim for loss of this dog by disappearance, theft, death or otherwise, and from any claim for damage or injury to the dog, whether such loss, disappearance, theft, damage or injury be caused or alleged to be caused by the negligence of CPDPA or MNCPPC.

I recognize and appreciate the dangers, hazards and risks involved in the use of the dog park and I agree that I have assumed such risks. On behalf of myself, my heirs, personal representatives, guardians, successors and assigns, I hereby release the CPDPA, its board of directors, officers, shareholders, agents, volunteers, employees and MNCPPC (collectively, the “Released Parties”) from, and agree not to sue the Released Parties for, any claims that I may have arising from or in connection with any physical, emotional or mental injury or property damage that I may suffer as a result of my use of the dog park from any cause whatsoever. I further understand and agree that any and all persons using the dog park by virtue of my permit shall be deemed to have assumed such risks and granted such release. In the event that I allow another person to use the dog park by virtue of my permit I shall advise such person of the requirement that he agree to assume such risks and I agree to indemnify and hold harmless the Released Parties from and against any claim which may be asserted by any such third party using my permit.

I have read all the rules and I agree to adhere to them. I understand that CPDPA reserves the right to revoke the permit(s) of any member who does not follow the rules of the dog park.
